How do I pursue this?
- After contacting Fulcrum Therapy, an initial interview is given over the phone to determine the probable suitability of this type of treatment.
- The therapist meets the applicant and family and assesses the applicant's physical and cognitive abilities and limitations and interest in pursuing this form of treatment.
- The intake assessment is filled out by the applicant, parent or guardian to provide further information. An important part of this intake form is a doctor's release that states that there is no restriction on this activity. In addition, evaluations from other therapists and IEPs can provide more background to determine eligibility and provide information about possible therapy goals.
- After the intake packet is completed, including release forms and additional reports, the therapist will contact the participant to discuss therapy goals and to schedule therapy times.
